    APRIL 4

    Bocata Arepa Bar, 10170 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager on staff; person in charge was unaware of the “big six” reportable foodborne illnesses and the associated symptoms; facility couldn’t provide an employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; observed an employee using a cell phone while handling food; an employee used front kitchen hand sink to wash dirty pans; front kitchen hand sink doesn’t have paper towels or hand soap; temperature of cooked arepas is too low; several items, including containers of arroz con pollo and a pitcher of salsa, are not date marked; menu doesn’t contain a consumer advisory for runny egg; several large containers of food are unlabeled; employee with a full long beard isn’t wearing a beard restraint; spatula near grill is bent; reach-in cooler under prep counter has a torn gasket and the bottom interior has food debris; no hot water is available at hand sink in back kitchen; ceiling panels in back kitchen are torn; ceiling light in back kitchen is broken; ceiling fan in back kitchen is dusty; floor tiles in front kitchen are cracked.

    APRIL 5

    Mission BBQ Richmond, 5440 Glenside Drive – The following violations were reported during a routine inspection: hand sink in dish area doesn’t have paper towels and the soap dispenser doesn’t work; a bottle and a pitcher found stored in separate hand sinks; sanitizer concentration at 0 ppm while dish machine was being used; sanitizer buckets stored on prep surfaces near food; employees working with food are not wearing the proper beard guards; observed buildup of dried food debris, dust and grease on surfaces of wire storage racks, racks inside warmer units, monitors and cables; cook line backsplash and caulking around prep tables are dirty; there are old labels and glue residue on clean containers; tile coving behind floor mixer is damaged; observed grease and food debris on floors throughout kitchen.

    Burger King, 5450 Glenside Drive – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit/diarrhea; dish drying racks, cooler door gaskets throughout kitchen and walk-in cooler wire racks are dirty; hoods above deep fryers are greasy.

    Bojangles, 5207 Brook Road – The following violations were reported during a routine inspection: front end hand sink doesn’t have paper towels; black mold found inside ice machine; temperature of liquid eggs on ice is elevated; bin of seasoned flour is unlabeled; cutting board in kitchen is scratched and stained; facility doesn’t have sanitizer test strips; observed food stains, grease and flour on walls behind and next to cooking equipment; sides of hot line cooking equipment are greasy; several items/areas are dirty (reach-in freezer door gaskets and exterior, biscuit area freezer and cooler door gaskets and interior, walk-in cooler racks, and dry storage rack).

    APRIL 8

    Lehja Restaurant, 11800 West Broad Street – The following violations were reported during a routine inspection: hand sink by cook line doesn’t have paper towels; raw shrimp is stored over onions and with other ready-to-eat food; observed multiple items in prep units and walk-in cooler and spices on dry storage shelf uncovered; observed mold buildup in ice machine and on soda nozzle; there is 0 ppm sanitizer in three-compartment sink when tested; dish machine isn’t reaching the required temperature; air freshener and men’s cologne are stored with food and clean dishes; bulk container of white substance is unlabeled; live roaches, small flies and mouse droppings found throughout kitchen; the cook is making and prepping food without wearing a beard guard; sanibucket has 0 ppm sanitizer; wet wiping cloths are stored on make tables instead of the sanibucket; a single-service container is being used as a rice scoop; fryer baskets are in disrepair; racks throughout are rusty; observed cracked containers and other visibly worn equipment and utensils on clean dish rack; observed grease on sides of equipment, buildup on shelves throughout, and lime scale buildup at dish area; drain under bar sink is backed up; there is low grouting; floor tiles are cracked; back door is rusty; ceiling above mop sink is peeling and moldy; cook line, under dish, and dry storage area need cleaning.

    Ironclad Coffee Roasters, 2100 John Rolfe Parkway – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures; handwash signage is not provided and/or not clearly visible at front hand sink used by food employees; temperature of breakfast sandwiches in warmer cabinet is too low; sliced ham in walk-in cooler is past its expiration date; deli ham isn’t date marked; employees in food service area are not wearing proper hair restraints; butcher block table is cracked, separating at edges and isn’t sealed; wood on pass-thru window isn’t sealed and is in disrepair; facility couldn’t provide sanitizer test strips; observed flour buildup and food debris on surfaces of storage racks, prep counters and milk crates; observed food debris on kitchen floor and in cooler; ceiling vents and panels are dusty; water heater is rusted at the bottom; ceiling panel near walk-in cooler corner is damaged; there is a gap in the bottom of the door that leads outside.

    Starbucks Coffee Company, 2363 Roux Street – The following violations were reported during a routine inspection: several items are dirty (ice chest in front area, cooler under coffee grinder, and wire racks in milk reach-in cooler); there is 0 ppm sanitizer in three-compartment sink when tested.

    APRIL 9

    New Top’s China 1, 7111 Staples Mill Road – The following violations were reported during a routine inspection: temperatures of pork, shrimp, sliced pork and garlic in oil in prep unit are elevated; prep unit is not maintaining foods at the proper temperature; bulk bins of salt, sugar and MSG are unlabeled; spices and sauces on cart near cook line are unlabeled; raw broccoli wasn’t washed thoroughly in water to remove contaminants before being prepared; to-go containers are stored unprotected on a dirty surface; observed an employee handle eggrolls while wearing cleaning gloves; cardboard is lining shelves and surfaces throughout the kitchen; walk-in freezer door and handles are in disrepair; wire racks in back storage room are rusted; observed food debris and dust on surfaces of walk-in cooler wire racks, inside reach-in cooler, and on cook line storage counters; there is a leak under the hand sink; walk-in cooler has inadequate lighting.

    Outback Steakhouse, 7917 West Broad Street – The following violations were reported during a routine inspection: dish room hand sink doesn’t have paper towels; yellow buildup found in soda gun at bar; temperatures of sliced tomato, shredded cheese and bleu cheese in make table are elevated; temperatures of shredded cheese, butter and bacon-cheese topping in reach-in cooler are elevated; temperatures of lettuce and shredded cheese in ice bath on line are elevated; observed an employee handling food while wearing a bracelet; plastic containers are stacked while still wet above prep sink; to-go containers stored on a dirty surface; long white cutting board at line and green cutting board in dish room are scratched and stained brown; fryer baskets are bent; microwave handles are loose; dish plate doesn’t reach the required temperature in dish machine at bar; monitors on the line, fryer cabinets and stand mixer are dirty; there are leaks at faucet and pipe at bar hand sink; floor tiles throughout kitchen and bar are cracked; grout throughout kitchen is low; walls throughout kitchen have chipping paint; wall at mop sink is in need of repair; ceiling fans throughout kitchen and fans in walk-in cooler are dirty.

    APRIL 10

    Subway, 3326 Pump Road – The following violations were reported during a routine inspection: facility couldn’t provide the employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; metal container found in hand sink; oil and vinegar bottles at the line are dirty; three shakers on the line are unlabeled; sanitizer level in sanibucket for wiping cloths is too low; metal containers are stacked while still wet near three-compartment sink; long white cutting boards at line have deep scratches and are stained brown; bottom of bread holder and racks throughout the kitchen are dirty; ceiling vent in walk-in cooler is leaking; ceiling fans throughout kitchen are dusty; mop isn’t being hung to air dry; floors are unsealed in walk-in cooler units; there is low lighting in both walk-in cooler units.

    Jersey Mike’s Sub Shop, 11444 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; temperatures of lettuce and sliced tomatoes in walk-in cooler are elevated (found in deep containers stacked full); temperatures of lettuce and tomatoes on line and sliced tomatoes and cheese at make table are elevated; temperatures of provolone, ham and peppered ham in display case are elevated; observed grime buildup on racks in dry storage area and above three-compartment sink, hood vents, suppression nozzles and wheels of grills; observed heavy buildup of dust on ceiling vent above line.

    APRIL 11

    Kabuto Japanese House of Steaks, 8052 West Broad Street – The following violations were reported during a routine inspection: food manager certification is expired; observed buildup in soda gun at bar and inside ice machine; dish plate doesn’t reach the required temperature in dish machine; fried rice in walk-in cooler isn’t date marked; dinner menu is missing consumer advisory for steaks; several bottles and bins of food are unlabeled; several boxes of food are stored directly on walk-in freezer floor; single-service containers are stored on dirty surfaces throughout the kitchen; dish racks, rusted wire racks and a cracked plastic bin containing zucchini are in disrepair; white cutting boards have deep scratches and are stained yellow/brown; cardboard is lining the walk-in cooler floor and wire racks; faucet at bar hand sink and pipe under three-compartment sink are leaking; several items/areas are dirty (wire racks throughout kitchen, exterior of dish machine, exterior of grease trap, top of ice machine, floors in dry storage room and at bar, floors and walls under dump sink at bar, ceiling panels throughout kitchen, walk-in cooler floor, wall behind dry storage racks in kitchen, and hood vents); ceiling vents and walk-in cooler fans are dusty; floor tiles in staff bathroom are cracked; ceiling vent above three-compartment sink is in disrepair; there is a hole in the wall behind dry storage room door; floor tiles in dry storage room are deteriorated; there is low grouting in dish room.

    Noodles & Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: spray hose attachment is soiled; several items/areas are dirty (shelf and legs on rack under oven, flat top grill support, wall and floor in mop room, pipes from dishwasher, floor under dishwasher, and walk-in door).
