    Polk County restaurant inspections: Dutch Bros, Domino's, La Cazuela

    ABOUT RESTAURANT INSPECTIONS

    This is a snapshot of the inspections conducted by the Polk County health department.

    Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

    Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.

    Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

    Priority violations :

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Bottle of shake mix is sitting out at room temperature (bottle is 55 degrees Fahrenheit). Operator states it was recently filled and has been out no more than 30 minutes. Point deduction: 5.

    Two Wolves Taproom

    Location: 171 SW Court St., Dallas

    Priority violations :

    The chlorine sanitizer concentration, pH, or temperature is not adequate, specifically: Chlorine sanitizer in dishwasher is zero parts per million. Bottle of sanitizer is empty. Point deduction: 5.

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Four containers of smoothie mix and one container of soft top are sitting out at room temperature. Operator states these have been out for about 10 minutes. Smoothie mix and soft top temperatures range from 62 to 64 degrees Fahrenheit. Point deduction: 5.

    Dude Donut City

    Location: 525 Taggart Drive Suite 130, Salem

    Priority violations :

    Hand towels or a hand drying device is not provided at the handwashing sink, specifically: Handwashing sink has no paper towels. Point deduction: 3.

    Ready-to-eat food is not properly date-marked, specifically: Opened container of half and half and milk are not dated. Operator states half and half was opened yesterday and milk was opened three days ago. Point deduction: 3.

    Food employees are not washing their hands in an approved handwashing sink, specifically: Operator washed hands under water without soap at three-compartment sink above pile of dirty dishes. No paper towels at handwashing sink in front area. Point deduction: 3.

    Food is in contact with surfaces of equipment and utensils that are not properly cleaned and sanitized, specifically: Basket of cilantro is sitting in handwashing sink. Point deduction: 5.

    Food-contact surfaces of equipment and utensils are not properly sanitized after cleaning, specifically: Operator washed blender at three compartment sink above pile of dirty dishes without sanitizing. Point deduction: 5.

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Mayo sitting on food prep unit is at 70 degrees Fahrenheit. Point deduction: 5.

    Habebah Mediterranean Cuisine

    Location: 110 Main St. W, Monmouth

    Date: March 14

    Score: 95

    Priority violations :

    Raw or ready-to eat food is not properly protected from cross contamination, specifically: Bags of marinating raw chicken are in top drawer under grill above falafel. No separation between drawers. Point deduction: 5.

    Domino's

    Location: 310 E Main St., Monmouth

    Date: March 14

    Score: 91

    Priority violations :

    Soap is not provided at a handwashing sink, specifically: Front handwashing sink has no hand soap. Point deduction: 3.

    The use of time as a public health control is not properly monitored, food is not properly marked or written procedures have not been developed, specifically: Times are not marked on red sauce, ranch and barbecue sauce. Operator states these were pulled from the fridge four hours ago and can remain at room temperature for up to seven hours. Point deduction: 3.

    A test kit is not provided or is not accurate enough to measure the concentration of sanitizing solutions, specifically: Facility has no quaternary test strips for three compartment sink. Point deduction: 3.

    Jack's Pub and Lotto

    Location: 423 Main St. E, Monmouth

    Date: March 15

    Score: 97

    Priority violations :

    Food-contact surfaces are not clean, specifically: Three dirty scoops are hung with clean scoops. These have oil, salt and sugar on them. Operator states these were likely dirtied the night before. Point deduction: 3.
