State orders South Hills nursing home not to admit new residents
The nursing home from which a wheelchair-dependent resident with dementia disappeared and was later found dead a few blocks away cannot admit new residents until it corrects violations. The Oregon Department of Human Services issued a “Restriction of Admissions” order to South Hills Rehabilitation Center on July 15. ODHS requires South Hills to post the restriction prominently on the inside and outside of its doors. The order comes after the ODHS determined the facility at 1166 E 28th Ave. “failed to provide appropriate care for residents with dementia,” according to a notice addressed to the facility. According to the notice, 67-year-old Carol Clove Petitti eloped from the facility twice in the span of three days, the second resulting in her death. After the first incident on July 3, staff did not properly reassess her risk and safety, nor did they give her proper supervision to prevent future elopements, ODHS determined. On July 4, Petitti drove her electric wheelchair into a flower bed and needed to be assisted out by a Hoyer lift. This incident, too, did not cause the facility to reassess her risk or safety. She left the facility again July 5 around 4:30 p.m., according to a Eugene
