Wellington Nursing Home Sued Over Woman’s Death, Sepsis And Malnutrition Alleged
WELLINGTON, FL (BocaNewsNow.com) (Copyright © 2026 MetroDesk Media, LLC) — The family of a woman who died last year is suing The Luxe at Wellington Rehab Center, claiming staff let her slide into sepsis, malnutrition and dehydration while she lived there. Deanna MacDonald, the daughter of Mary Olsen and the personal representative of her estate, filed the lawsuit in Palm Beach County Circuit Court. It names Wellington Rehab and Health Center, LLC, the facility’s licensee and owner. Olsen died May 20, 2025. According to the complaint obtained by BocaNewsNow.com and published below, staff never built an adequate care plan for Olsen and failed to watch for warning signs as infections, weight loss and dehydration set in. The suit says the facility didn’t alert her family or doctors to those changes, didn’t follow physician orders and didn’t get her to a hospital in time. It also blames inadequate staffing, poor training and poor supervision of employees. The case is brought under Chapter 400 of Florida law, which spells out the rights of nursing home residents. The family is seeking more than $50,000 in damages and a jury trial. Olsen’s survivors are listed as MacDonald, Edward Olsen Jr. and Stephanie Brick. The

Boca Raton Nursing Home Sued Over Resident’s Death From Severe Bedsore
BOCA RATON, FL (BocaNewsNow.com) (Copyright © 2026 MetroDesk Media, LLC) — A Boca Raton nursing home let a resident’s bedsore deteriorate until bone was exposed, then failed to properly care for the wound until he died, according to a wrongful death lawsuit filed in Palm Beach County Circuit Court. Mandana “Mandy” Kalliche Weber, personal representative of the estate of Robert Bernard Kalliche Weber, filed the suit Oct. 22 against Avante at Boca Raton, Inc., which operates Avante of Boca Raton SNF. Weber lived at the facility from Sept. 28, 2023, until his death on May 29, 2025. The complaint describes him as immobile and fully dependent on staff for repositioning, hygiene, nutrition and wound care. By January 2024, Avante had documented a Stage III pressure ulcer on his sacrum. By November, the suit says, the wound wasn’t healing and bone was visible. He was sent to the hospital Dec. 11, 2024, where the wound was diagnosed as a Stage IV pressure injury with cellulitis. He underwent debridement and a diverting colostomy on Dec. 27. When he returned to Avante in January 2025, the wound measured roughly 11.5 by 9 centimeters and 2.5 centimeters deep. The lawsuit alleges that from February
