Indiana DMHA, Praxis Landmark Recovery reach settlement
MISHAWAKA, Ind. (WANE/AP) — An embattled chain of addiction recovery centers has reached a settlement with the Indiana Division of Mental Health and Addiction (DMHA). The DMHA, a division of the Indiana Family and Social Services Administration, also revoked the licenses of three Praxis Landmark Recovery locations in Bluffton, Mishawaka and Carmel in 2023 following three deaths at the Mishawaka center and one death from someone last seen alive at the Bluffton facility.
