    Observed: Live cockroaches throughout kitchen areas by outlets, cooler motors, and in glue traps. Facility needs to constantly be monitoring for cockroaches and find ways to minimize their presence. Pest control reports have been requested. A re-inspection will occur.

    Observed: A live cockroach crawl across the sushi counter cutting board. Sushi cutting board and knife were removed to be cleaned. Any open food items located on sushi counter were voluntarily discarded. A re-inspection will occur.

    We made a rough calibration of metal stem thermometer, which was reading 60 degrees F and mine was reading 72 degrees F. He did not have any ice to properly calibrate thermometer. A reinspection will be made on or after Monday, May 6th to check refrigerators.

Person in charge corrected by marking for time as a control and will discard at the end of 4 hours. Discussed moving potentially hazardous foods into prep cooler for easier access. Person in charge agreed to make that change in the future.

    Observed a case whole eggs, and a case of tofu above 41F in non-functional walk-in cooler. Person in charge was aware unit was non-functional and had a call in to repair person, but it would be a few days. Corrected on site by discussing that no foods that require temperature control will be stored in non-functional coolers and voluntarily discarding foods. This is third of this violation, risk control plan created on site with person in charge.

    Observed floor cleaner in soy sauce bucket, not marked to indicate the liquid in bucket. Person in charge corrected on site by discarding.
