Social service work often involves entering private homes and witnessing uncomfortable living conditions and unmet basic needs. The Care Coordinators at Senior Services develop positive ways to approach this difficult work. I write this column each month to address the needs of older adults. As I retire from my position this month, I would like to address the practitioners. I wish I could convey the remarkable ingenuity, patience, endurance and courage I see every day in the staff of Care Coordination. These trained individuals fulfill the call to serve older adults in times of need with diligence, compassion and skill. The work begins with a phone call to establish a time for an assessment in the older adult's home. The drive to the home provides an understanding of the level of support — or isolation — in the living environment. The work of a Care Coordinator is not only procedural and technical but also relational, emotional and intellectually demanding. Observing an older adult in their home environment, noticing tones and body language, and sensing hesitation, exhaustion and isolation are part of the authentic human connection that allows for the development of a meaningful and effective care plan. Senior Services Care Coordination Corner: Reevaluation

Reevaluation means looking at something again to determine whether plans, rules or conditions need to change. Finances, living environments and health change over time, and so must people’s relationship to them. People often talk about living beyond their means. Usually, that means spending more money than they earn. But it also can mean remaining in a living environment that no longer supports their life. Perhaps a person lives in a multilevel home and can no longer use stairs safely. Perhaps home repairs and recurring maintenance are no longer affordable. Perhaps a care partner arrangement no longer meets a person’s needs. These also are examples of living beyond one’s means. As with living beyond financial means, the result can be constant stress and worry that affects overall well-being. There may come a time when a person needs to walk away from something that is no longer working, creating both necessary endings and positive beginnings. The Senior Services Care Coordination program provides guidance and resources to help people live within their means. Even small steps can lead to meaningful progress.
