Trillium Health Resources offers tailored plans

Trillium Health Resources is a major component in Martin County’s healthcare initiatives and focuses on one of the state’s major health problems, mental health.

On July 1, the organization will begin offering tailored plans for members.

During Fiscal Year 2022-2023 over 40,000 individuals with severe behavioral needs were served by Trillium Health Resources (THR) in the five regions supported by Trillium, according to Regional Vice President Bobbie Lowe.

Trillium is a local government agency managing mental health, substance abuse and intellectual/development disability services in a multi-county area in eastern North Carolina.

Of the 43,571 individuals served, 32,939 had mental health issues, while 12,526 had substance abuse problems and 7,456 had Intellectual Developmental Disabilities. The cost for these services was $653,498,469.

In Martin County, a total of 806 consumers were served. Of those, mental health issues challenged 624 residents, 182 suffered from substance abuse and 136 had intellectual developmental disabilities.

Trillium has made changes in their care management programs improving service and expanding provider based tailored care management. Plan based tailored care management programs have developed new specialized teams with focus on whole person care, explained Lowe.

The programs that have been updated include substance abuse prevention in schools, the naloxone kits program, summits to discuss mental health challenges and family solutions.

The N.C. Department of Health and Human Services (NCDHHS) announced that the Behavioral Health and Intellectual/Developmental Disabilities (I/DD) Tailored Plans will launch on July 1.

As N.C. Medicaid continues its transition to managed care, the focus is on building effective models for managing members’ comprehensive needs through care management. N.C. Medicaid is investing in the Tailored Care Management model, a community-based, provider-driven model aimed at whole person care and driving toward better health outcomes.

Through Tailored Care Management, Tailored Plan members will have a single designated care manager supported by a multidisciplinary care team to provide integrated care management.

“Tailored plans are specifically designed to serve the complex needs of individuals with significant behavioral disorders, IDD and traumatic brain injuries. The tailored plans will also serve uninsured individuals receiving state-funded services, regardless of their diagnosis along with those remaining in N.C. Medicaid Direct,” said Lowe.

Beneficiaries who will be covered by the Trillium Tailored Plans will continue to receive behavioral health, I/DD, TBI and physical health care as they do today. The Trillium Tailored Plan will also cover pharmacy services for members in the plan.

“While Trillium currently coordinates services for behavioral health and intellectual/developmental disability needs, with the launch of the Trillium Tailored Plan, we will have broader capabilities to serve members including physical health care and pharmacy needs,” said Lowe.

The launch of Trillium’s Tailored Plan program offers a variety of new changes that will benefit members.

Member and recipient services will be available on Saturdays as well as Monday through Friday. Trillium will help schedule non-emergency medical transportation for members to get to doctors appointments.
