Richland county restaurant inspections: critical violations

Ontario Duchess #1249, 3675 Park Avenue West, Ontario, April 3. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical repeat). Multiple critical violations observed during inspection. Correct by April 12. TCS (time/temperature control for safety) foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed multiple TCS foods held in deli grab and go coolers to be held with internal temperatures above 41 degrees for an undetermined amount of time. PIC voluntarily discarded all TCS foods held above 41 degrees. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Observed opened bags of Stok cold brew in self-serve retail dispenser not date marked. Employee unable to verify when bags were opened and discarded during inspection. Food contact surfaces not easily cleanable (critical, corrected). Observed scissors used to cut creamer and milk bags as well as bags of coffee not easily cleanable as scissors cannot be fully separated. Employee removed scissors from use. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed drain from ice maker on top of fountain beverage machine in retail area not air gapped at floor level rim of floor drain. Correct by April 12.

Madison High School, 600 Esley Lane, Mansfield, April 3. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). One employee had not signed agreement.

Jimmy's Stop N Go LLC, 753 Springmill St., Mansfield, April 5. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC could not state safe temperatures. Education was provided. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Observed employee unable to locate employee illness agreements. Ensure these agreements are easily locatable. PIC signed at time of inspection. Foods from unapproved sources (critical, corrected). Observed Royal Honey VIP products for sale within the facility. Since facility is unable to determine if food or supplement, and as per guidance with the Ohio Department of Agriculture this is from an unapproved source. PIC removed all product from sale at time of inspection. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed spray bottle of Windex on the prep surface in the kitchen, and Fix a Flat over a cooler in the drive thru. Ensure all chemicals are stored completely separated from food and food prep surfaces.

Ducky's Pub And Grill, 9 Cleveland St., Butler, April 5. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed walk-in cooler ambient air at 45 degrees. Internal temperatures ranged from 44 to 47 degrees. PIC voluntarily discarded all TCS products. Service company to be on site April 6 for repairs on unit.

Penn Station, 2166 Walker Lake Road, Mansfield, April 5. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected during inspection). Observed bag of sugar on shelf in back storage room open and uncovered/unprotected from potential contamination. Improper use of time as a public health control - 4 hours (critical, corrected). Observed container of blanched fries stored at room temperature by fryer not time stamped to indicate 4 hour hold time/time of discard. PIC verified when fries were pulled from cooler and had employee time stamp fries during inspection. Insufficient air gap between the flood rim and the water supply inlet (critical repeat, corrected). Observed disposable cup placed into floor drain, eliminating air gap for drain lines from fountain beverage machine and ice maker at floor drain located inside cabinet below machine. PIC removed cup to allow for a proper air gap.

Kimmyscucina, 275 E. Main St., Lexington, April 9. No safe handling instructions on packaged raw meat (critical, corrected during inspection). Pork chops, bacon and Italian sausage in the retail freezer were missing safe handling instructions. PIC removed pork chops and bacon from retail freezer and labeled Italian sausages.

Carpenter Y Day Care, 71 Carpenter Road, Mansfield, April 9. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed a spray bottle of bleach water that was not labeled. PIC labeled bottle with contents at the time of inspection.

Mansfield Senior Culinary Arts, 124 N. Linden Ave., Room M1801b, Mansfield, April 9. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Can opener blade had visible dried residue. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). Tomato salad and milk opened for over 24 hours observed. PIC voluntarily discarded tomato salad and date marked milk. Food contact surfaces not easily cleanable (critical, corrected). Scissors observed that could not be separated in clean area. PIC removed from service. Food contact surfaces not easily cleanable (critical repeat, corrected). Salad tub in clean area observed with crack in food contact area that rendered it no longer easily cleanable. PIC removed from service.

Pleasant Hill Lake Park - Camp Store, 3431 Ohio 95, Perrysville, April 9. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Windex spray cleaner observed stored adjacent to unprotected tableware. Listed item was relocated to chemical storage area away from food storage.

Main Street Pit Stop, 56 W. Main St., Lexington, April 9. Food display not properly protected from contamination by consumers (critical, corrected during inspection). Observed a basket of apples in the cooler that were not covered to protect from contamination. Discussed wrapping apples or posting signage indicating to wash apples prior to consumption. PIC removed apples from sale.

La Mexicana Market, 1329 Lexington Ave., Mansfield, April 10. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Observed hand soap and laundry detergent over foods for sale in the back storage room. Ensure all chemicals are stored completely separated from items for sale.
