Toro, 39, of Jupiter, Florida, operated Reawakenings Wellness Center (RWC) and submitted fraudulent insurance claims for substance abuse treatment that was never provided to patients, including FEHBP beneficiaries. Using personal identifying information of former RWC patients, Toro obtained nearly $4.2 million in proceeds from the FEHBP through the submission of fake claims.
