The roadmap must identify which commercial plans, if any, the center currently accepts, how it bills, who qualifies for care, and where gaps remain in its provider network, the outlet reports. It also has to name at least one lab option that could realistically launch soon — including who would operate it, what it would cost, what approvals are required, and a possible start date.</p><p>That matters because San Mateo Medical Center, which the Coastside Health Center is part of, states plainly on its billing page that it does not participate in any commercial insurance networks. The medical center's site lists Health Plan of San Mateo coverage — including Medi-Cal, Care Advantage, and HealthWorx — among the plans it does accept, but nothing from the commercial side.</p><p>Any lab collection program that comes out of the roadmap would be open to Coastside residents regardless of their insurance, including people whose doctors practice outside the county health system, the same account notes. Currently, Coastside residents often have to travel over the hill to the bayside for routine blood draws, diagnostic testing, and follow-up appointments tied to medication monitoring.</p><h2>A Pilot Program's Shadow Looms Over the Decision</h2><p>The push follows a county-supported urgent-care pilot that ran out of a Half Moon Bay clinic from late October 2024 through mid-April 2025, with some appointments continuing into June. The trial was meant to fill the gap left by the April 2024 closure of AHMC Seton Medical Center Coastside, as Coastside News reported. It logged 1,638 appointments, with 82% involving Coastside residents, and it accepted patients regardless of insurance status, including people with none at all.</p><p>Commercial insurance turned out to be the largest payer category among pilot patients, ahead of Medi-Cal and Medicare — a detail that underscores why supervisors are now pushing the clinic to figure out commercial billing. When the pilot wound down, Dignity Health said the office would stay open for primary care and keep offering the same services that had been available under urgent care.</p><p>That didn't last. Dignity Health went on to announce its Half Moon Bay clinic would close effective October 31, 2025, citing what it called ongoing challenges recruiting the medical providers needed for sustainable patient access, according to a separate report from the same outlet. Coastside News noted the closure came four months after the county-supported urgent-care pilot had already ended.</p><h2>Supervisors Frame It as an Access Issue</h2><p>For Mueller, the stakes are about who gets left out. Seniors, people with access and functional needs, and Coastside families should not be isolated from routine preventive and accessible urgent care, he said, according to the dossier of county statements tied to the resolution. The Coastside Health Center, at 225 S. Cabrillo Highway, currently offers primary care for Medi-Cal beneficiaries and uninsured adults, teens, and children, along with family planning, women's health, mental health, and dental services.</p><p>San Mateo County Health Director Colleen Chawla said the department would deliver feasible roadmap options while protecting access for Medi-Cal, Medicare, uninsured, and underinsured patients. County Health is expected to develop the roadmap alongside the County Executive's Office, the County Attorney, and other county departments, and any plan it produces must comply with federal Health Center Program requirements — standards the Bureau of Primary Health Care lays out in its own Health Center Program Compliance Manual, which health centers use to understand what federal compliance requires.</p><p>That federal guidance includes specific expectations around billing. A separate chapter of the manual states that health centers should keep billing records showing they submit claims in a timely and accurate manner to the third-party payors they participate with, consistent with their contracts — a baseline standard, not a judgment on San Mateo County's specific situation.</p><p>A staff report accompanying the resolution stated the fiscal impact of the directive itself as none, since it only orders a study rather than funding new services. The Midcoast Community Council had submitted a letter in September supporting expanded commercial insurance acceptance at the Coastside Health Center, adding community pressure ahead of Tuesday's vote.</p></div></body>
