Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Nov. 23 and Dec. 1:

Butler Duchess #1209, 11 Main & Elm, Butler, Nov. 23. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand wash station being used for storage of cleaning chemicals and beverage cans. Improper storage of poisonous or toxic materials (critical, corrected). Cleaning spray located beside beverage cans.

Madison Middle School, 1419 Grace St., Mansfield, Nov. 28. Handwashing sink not accessible (critical repeat, corrected during inspection). Food cart was blocking way to hand wash sink. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). RTE pickles observed displayed on food line above sneeze guard and was subject to sneeze, splash, dirt or dust.

Panera Bread # 4787, 1402 Lexington Ave., Mansfield, Nov. 28. Person in Charge (PIC) unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). Observed PIC unable to demonstrate proper cooling and reheating times and temperatures. Education provided at time of inspection, also poster for cooling and reheating is included in emailed report. PIC unable to demonstrate knowledge by having no critical violations (critical). Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed no lids on the topping containers.

Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed production build-up on the knives. TCS foods not being cold held at the proper temperature (critical, corrected). Observed the following items to be outside of appropriate temperatures for cold holding: four packages of cut fruits; five containers of orange juice; and 14 six-pans of sliced tomatoes. PIC had all items voluntarily discarded. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed no gap on the drain in the drive thru area soda machine. Correct by re-inspection Dec. 5.

Everbowl, 2074 Walker Lake Road, Mansfield, Nov. 29. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed a container of house made everoats in front reach in cooler to have internal temperature of 46 degrees at time of inspection. PIC states oats were made previous day and were voluntarily discarded.

Pulp Juice and Smoothie Bar, 994 Lexington Springmill Road, Ontario, Nov. 29. Person in charge (PIC) not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical). PIC unable to demonstrate knowledge of cleaning and sanitizing (critical). PIC unable to demonstrate knowledge by having no critical violations (critical). Observed multiple critical violations during inspection. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed washed fruit and cut vegetables in reach-in cooler below juicer machines not covered/protected. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected).

Observed dry mixes and ice bin along prep counter open when not in use, no customers present in facility. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed sherbet and frozen yogurt as well as frozen fruit in freezer along make line to be uncovered/unprotected at time of inspection. Equipment and utensils contacting TCS food not cleaned every four hours (critical, corrected). Observed pitchers contacting TCS (time/temperature controlled for safety food) only rinsed and not cleaned when required. Ensure food contact equipment and utensils that are in contact with TCS foods are cleaned at least every four hours throughout the day. Presence of live insects (critical).

Observed multiple gnats around floor drain below three-compartment sink and around rubber mat where pitchers drain. Correct by Dec. 6. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed wash solution in three-compartment sink at 96 degrees.

Lucas Elementary & High School, 84 Lucas Road N., Lucas, Nov. 29. Improper washing of hands and arms (critical, corrected during inspection). Food worker observed touching contaminated sink faucet handle with clean hands.

Gwirtz Foods, 5001 Ohio 39, Shelby, Nov. 30. Improper bare-hand contact with RTE foods (critical, corrected during inspection). Employee contacting ready-to-eat cheese with bare hands. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Sanitizer in bucket was in excess of 400 ppm. Improper storage of poisonous or toxic materials (critical, corrected).

Chemicals observed stored next to and above single use items in produce packaging area. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Sanitizer bucket was not labeled with common name. No air gap or approved backflow prevention device on the plumbing system (critical, corrected). Spray hose in smoker room was not equipped with back flow. PIC is to have backflows installed by re-inspection.

Eastview Elementary, 1262 Eastview Drive, Mansfield, Nov. 30. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). True two-door cooler observed and measured at 46 degrees. Single pint of of dairy milk was voluntarily discarded.

Tim Hortons, 1072 Ashland Road, Mansfield, Dec. 1. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed improper air gap under three-compartment sink. Pipe shall be cut to prevent back siphonage of water.

Barnes & Nobles Booksellers Inc. #2731, 832 N. Lexington Springmill Road, Mansfield, Dec. 1. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer solution in sanitizer bucket to below 200 ppm.

Sol Froyo, 2170 Walker Lake Road, Ontario, Dec. 1. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). No employee illness report agreements observed for food employees working during inspection. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed cut strawberries in cold holding unit in customer self serve area to have internal temperature of between 42 and 44 degrees. PIC placed cut strawberries in walk-in cooler to cool to 41 degrees,

Butler Elementary School, 125 College St., Butler, Dec. 1. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Employee was taking temperature without first sanitizing probe thermometer. TCS foods not being hot held at the proper temperature (critical, corrected). Single portions of hash browns on serve line stored outside of steam table measured at 129 degrees. PIC voluntarily discarded four single portions of listed food. TCS foods not being hot held at the proper temperature (critical, corrected). TCS egg omelettes measured on serve line outside steam table at 109 degrees . PIC voluntarily discarded listed food.

Cornerstone Bar & Grill, 586 Park Avenue East, Mansfield, Dec. 1. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed cutting block that has deep gauges in board. PIC volunteered to replace board.
