Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between Sept. 20 and Sept. 21:

Tito's Cantina and Grill, 320½ E. Main St., Lexington, Sept. 20. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employee present did not have record of being informed of responsibility to report. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Raw meats and eggs observed above ready-to-eat foods in walk-in cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Pico observed left out on counter that was not in preparation and uncovered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Tilapia observed thawing in walk-in cooler uncovered. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Person in charge (PIC) separated into smaller pans and put in ice bath and stirred to rapidly bring temperature to below 41 degrees. Mocha hot sauce made in-house in make table had an internal temperature of 44 degrees for an undetermined amount of time. PIC voluntarily discarded one pint. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Spray bottle per PIC to contain bleach was not labeled with common name.

TGI Friday's, 900 Lexington-Springmill Road, Mansfield, Sept. 20. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Ice bin in bar was observed left open with ice subject to sneeze, splash, dirt and dust. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Tongs hanging above drying rack were observed with food residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Probe thermometer of food employee was going to be put into RTE tomatoes before being stopped by inspector. Thermometer was sanitized. TCS foods not being cold held at the proper temperature (critical, corrected). Lower reach-in cooler on cook line near toaster had ambient temperature of 49 degrees. Ribs were removed from cooler and relocated. Dressing was voluntarily discarded.

Mansfield Fuel & Food Mart LLC, 3880 W. 4th St., Mansfield, Sept. 20. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Multiple critical violations observed. Education provided for critical violations throughout inspection. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed large pizza spatula near pizza oven to contain visible dried build up. TCS foods not being hot held at the proper temperature (critical, corrected). Observed fried chicken pieces and jalapeno poppers in hot hold retail case held below 135 degrees. Employee discarded. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed containers of egg/sausage scramble to prep cooler to not have date mark. Improper use of a poisonous or toxic substance (critical, corrected). Observed can of Raid on counter next to food prep cooler. Presence of live insects, rodents and other pests (critical). Observed rodent droppings and small flying insects in retail self-service coffee area. Thoroughly clean area and monitor for further activity by Sept. 27.

Five Guys Burgers & Fries, 994 N. Lexington-Springmill Road, 160, Ontario, Sept. 21. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Employee on shift did not have employee reporting agreement signed. Improper washing of hands and arms (critical, corrected). Male employee observed touching contaminated sink handles. TCS foods not being cold held at the proper temperature (critical, corrected). Lettuce in bottom cold holding unit by register measured at 47 degrees. TCS foods not being cold held at the proper temperature (critical, corrected). Tomatoes in upper cold holding unit was measured at 52 degrees.

The Cove Coffee Shop, 325 Shelby Ontario Road, Ontario, Sept. 21. Chemical sanitizer other than chlorine, iodine, or a quaternary ammonium applied improperly (critical, corrected during inspection). Observed working container of sink and service cleaner under strength (0 ppm). Improper storage of poisonous or toxic materials (critical, corrected). Observed multiple spray bottles of cleaner hanging on side of cart right next to clean/sanitized food utensils near three-compartment sink in coffee area. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed wash water in three-comp sink in pizza area to be at 104 degrees. Ensure wash water maintained for manual warewashing is at a minimum of 110 degrees.

Mansfield/Ontario Fairfield Inn & Suites, 1065 N. Lexington Springmill Road, Mansfield, Sept. 21. PIC did not ensure employees are properly maintaining temperatures of TCS foods during hot and cold storage (critical, corrected during inspection). Education regarding safe cold holding of 41 degrees or below and safe hot holding of 135 degrees or above provided during inspection. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected). Education provided for critical violations. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). PIC unable to locate employee illness agreement for current food employee working. Copy of illness reporting agreement provided and signed. Observed TCS foods along breakfast self-service counter not held at 41 degrees or below. PIC discarded listed food items. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed opened bags of shredded cheddar cheese in cooler without date mark. Improper storage of poisonous or toxic materials (critical repeat, corrected). Observed bottle of multi-surface cleaner hanging from wire storage shelf above single service cups and utensils in kitchen area. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed wash water in three-compartment sink to be at 107 degrees.

Rancho Fiesta, 1360 S. Trimble Road, Mansfield, Sept. 21. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a pan of raw chicken above ready-to-eat food on a rolling cart inside of the walk-in cooler. TCS foods not being cold held at the proper temperature (critical, corrected). Observed a pan of shredded cheese sitting on the make table counter at 58 degrees. PIC voluntarily discarded cheese. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed a spray bottle of bleach water that was not labeled.

Arby's Cam/Rb Inc., 2325 Interstate Circle, Mansfield, Sept. 21. Handwashing sink not accessible (critical, corrected during inspection). Observed disposable wiping cloth located in front hand wash sink.

Leaning Tower of Pizza, 180 Lexington Ave., Mansfield, Sept. 21. PIC unable to demonstrate knowledge by having no critical violations (critical). Single-use gloves used improperly (critical). Observed employee with gloves on open the walk-in cooler, take out a tomato and attempt to go back to work without removing gloves and washing hands. Single-use gloves used improperly (critical, corrected during inspection). Observed an employee using a towel from his shoulder to wipe his face with his gloves on and started to go back to work without changing his gloves. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Sani bucket had chlorine in it way above 100 ppm. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed can opener with old food build-up on the blade. Improper storage of poisonous or toxic materials (critical, corrected). Chemicals stored on shelf above sink for water for making dough. Presence of live insects (critical repeat). Observed flour beetles, gnats and flies in the facility. Re-inspection Sept. 28. Presence of live rodents (critical). Observed mouse droppings in closet and back storage area. Contact pest control company. Re-inspection Sept. 28. Food contact surfaces not easily cleanable (critical). Cutting table is chipped and holding production build-up. Replace by next standard inspection.
