A 22-year-old man has died days after he was involved in a crash while working at an Amazon warehouse outside Carlisle, authorities said.
Alex Carrillo, of New Oxford, died Saturday at Holy Spirit Hospital, according to Cumberland County Coroner Charley Hall. Carrillo’s death was ruled accidental as a result of multiple traumatic injuries.
An Amazon spokesperson said Carrillo was on a powered industrial truck at a fulfillment center at 2 Ames Drive in South Middleton Township when another truck crashed into his Aug. 1.
Carrillo’s death marks the third Amazon employee death at a Carlisle-area warehouse since 2014.
“Our thoughts are with him and his family during this difficult time and we stand ready to support them however they may need it,” said Amazon spokesperson Sam Stephenson.
Stephenson said Carrillo was the only employee injured in the crash.
Amazon is investigating the crash, alongside the Occupational Safety and Health Administration (OSHA).
Devan Michael Shoemaker, 28, died at the South Middleton Township warehouse at 675 Allen Road in September 2017 while assisting the driver of a truck tractor in hooking up to a trailer, when the tractor driver accidentally ran over him, Hall said.
In June 2014, 52-year-old Jody Rhoads, of Carlisle, had been driving a pallet truck and crashed into some shelves at the Allen Road warehouse, according to the coroner’s office.
An OSHA spokesperson said the agency has six months to complete its investigation into the crash that caused Carrillo’s death. The spokesperson declined to provide additional details on what caused the crash, citing the active investigation.
