The King's Affection is a gender-swap TV series. Inside the royal palace, the princess gave birth to twins, a boy, and a girl. However, twins are considered an ominous sign. The king believes the little girl is "an abomination." So, he orders the death of his granddaughter.

The new mother found a way to trick the king's assassin. She told him the little girl was already dead. When the killer looked at the newborn, she didn't move and looked dead, so he left.

However, when the mother and her cohorts pulled a needle in the little girl's neck, she started breathing again. The baby girl was secretly taken away from the palace.

After several years, young Lee Hwi (Nam Yoon Su), the royal prince, noticed that one of the servants, Yeon Seon (Park Eun Bin), looked just like him. He felt that they could swap roles, and they did several times. But in a twist of fate, someone went after the girl and ended up killing the crowned prince.

To hide the actual prince's death, the mother summoned her daughter and had her pretend as the crown prince. She keeps herself distant from most people but will find herself falling in love with Jung Ji-Woon (Rowoon).
