The Tulsa Health Department is providing guidance for those who've lost their COVID vaccination cards.

The THD replacements only apply to vaccines entered in the Oklahoma State Immunization Information System, which doesn't include all doses administered by federal and tribal entities. If you received the vaccine at a tribal facility, health officials say you can contact the facility and request that your information be registered with THD.

To get a replacement, THD says to go online and print a copy of your shot records. To access your records, click here.

If the search cannot locate your records and you know you received your vaccine through THD, officials say to make sure all your provided information, like name, date of birth, and phone number/email, match the form you filled out when you first received the vaccine.
