OSHA was notified of the fatality by the Chattanooga Fire Department.

After investigating, OSHA decided to cite the Railroad Museum because of the many safety violations.

OSHA stated that Walker fell through a skylight, 37 feet to the ground.

Walker was working on the roof, caulking holes to stop a leak. OSHA stated that Walker did not have on fall protection.

They cited the Railroad Museum a total of $12,000 from three separate violations within the citation.
