TULSA, Okla. (KTUL) — The Tulsa Health Department is providing guidance for those who've lost their COVID vaccination cards. The THD replacements only apply to vaccines entered in the Oklahoma State Immunization Information System, which doesn't include all doses administered by federal and tribal entities. If you received the vaccine at a tribal facility, health officials say you can contact the facility and request that your information be registered with THD.
